    *If you are registering multiple persons at one time, you will be prompted to enter the full contact information for the other paid registrants when the payment process is completed.

    Please be advised that only paid attendees will be allowed in the conference room. Children and unregistered spouses may not attend the conference or sponsored events. Name badges must be worn at all times.

    Registration Policy: Each attendee must create an account and purchase their own registration. This step will enable the attendee to access course materials, and to retrieve their CME certificates.

    Cancellations must be received in writing no later than September 1, 2016 to be eligible for a refund, less a $150 cancellation fee. Cancellations received after September 1, 2016 and before October 1, 2016 are eligible for a refund less a $200 late cancellation fee. Cancellations after October 1, 2016 and before November 1, 2016 are eligible for a refund less $300. No refund will be issued for cancellations after November 1, 2016. Registrations are non-transferable. Failure not to appear at the program with no prior written cancellation will result in complete forfeiture of the conference registration fee and no CME credits will be issued with no exceptions. Cancellation requests must be received in writing by email or fax at 678-443-4029. You must receive a written acknowledgement to your written cancellation.
